Online Depression, Anxiety and Stress scale questionnaire DASS-21
Welcome to the Depression, Anxiety and Stress Scale questionnaire, known as the DASS- 21. This shorter version is a self reporting questionnaire, it measures the severity of a range of symptoms that are common to depression, anxiety and stress.
Not only will it provide a view of the level of emotional distress you may be experiencing and in which particular categories, but it will also act as a measure of your response to our treatments as your progress through therapy.
As depression, anxiety and stress are closely related, your range of scores will provide us a further opportunity for specific targeted interventions to address any new information or pre existing conditions that we may uncover and become aware of during therapy.
This questionnaire is used as an informative process and in this particular context it is not used for any diagnostic purposes. Please consider your responses based on what you have experienced over the last week in relation to the questions.
By answering all the questions as accurately and honestly as possible, this will ensure that the clearest score is produced from your responses. Selecting to respond with your initials or first name and email address, you will receive a direct email with your range of score explaining the ranges.
You also have the choice to respond anonymously, and no personal data will be stored by our company.
